Data you give us: Personally identifiable Information refers to information that tells us specifically who you are, such as your name, email address, or phone number.
Among the types of personal data that HealthForce collects are: first and last name; email address; and specific job application and employment information submitted via Formstack/Webmerge forms (see “Offsite Links” section below), when an application or other form is filled out.
HealthForce collects personally identifiable data only for purposes of doing business with its employees and customers, and does not sell or share this data except where required by law.
HealthForce may share this data with our Client Companies for the purpose of determining an applicant’s eligibility and suitability for assignments with that Company. Some of the specific employment forms, like the Criminal Background Affirmation include an acknowledgement that this information may be shared. Please read each form carefully as you complete the application.
Applicants who wish to opt out of any sharing of their data with Client Companies should contact our office and request to apply offline. These applications may be completed in person, by email or by regular mail. Applicants may specify their preferences and restrictions to the sharing of their data during this process.
Data from using the HealthForce site: We automatically collect information each time you visit the site. This usage data includes technical information (IP address, browser type and version, time zone settings, browser plug-in types and versions, operating system, platform, etc.), information about your visit (URLs, clickstream data, navigational data, page response times, download errors, length of visits to certain pages, and location data (country location by IP address, etc.).
